Hamilton Medical Consultants includes a group of multi-specialty physicians and advance practice providers with significant experience in high-acuity inpatient care, outpatient and post-acute care. We collaborate with multiple sub-specialists to produce the highest level of post-acute care. We are skilled to provide the continuum of care each patient requires on his or her path to recovery, from the hospital setting to the post-acute transition.

Those patients that are in facilities for skilled nursing/rehab care have a lower risk of readmission to the hospital and and have a shorter length of stay. Residents who are in the long-term setting receive regular and detailed management of their medical needs. HMC providers are regularly available on-site for patient rounding and provide 24/7 on-call coverage for each patient’s medical management. HMC providers collaborate with each facility’s leadership team to engage in quality programs and education to the nursing and facility staff.

Dr. Anzaldua attended St. Mary’s University in San Antonio, Texas, for his undergraduate studies. He obtained his medical degree from Saba University School of Medicine, then completed his Internal Medicine residency at St. Vincent Hospital in Indianapolis, Indiana. Following residency, he completed his General and Interventional Nephrology Fellowship at St. Louis University in St. Louis, Missouri. He is board certified in Internal Medicine and Nephrology by the ABIM.
Dr. Anzaldua is the owner of Kidney Professionals of Indiana and Hamilton Medical Consultants. In addition to his work with Kidney Professionals of Indiana, Dr. Anzaldua serves as the Medical Director at Indiana Veterans’ Home in West Lafayette, at Evergreen Crossing & The Lofts in Indianapolis, and at Green House Cottages in Carmel.
Dr. Armstrong attended Earlham College in Richmond, Indiana, for his undergraduate studies. He obtained his medical degree from Indiana University School of Medicine, and followed that with a family practice internship with Indiana University Medical Center. He did his Internal Medicine residency at St. Vincent Hospital. He has held board certifications in Internal Medicine and Geriatric Medicine.
Dr. Armstrong has specialized in Internal Medicine since 1987. He currently serves as a hospitalist at St. Vincent Hospital in Indianapolis. He has managed patients in the nursing home setting his entire professional career. Currently, Dr. Armstrong serves as Medical Director at Harcourt Terrace Nursing & Rehabilitation and Spring Mill Meadows.
Senior Adult Nurse Practitioner
Ms. Arnett obtained Indiana University and obtained her Bachelor of Science in Nursing. She graduated with her Masters of Science in Nursing and an Adult Nurse Practitioner degree from Indiana University-Purdue University, Indianapolis. Ms. Arnett specializes in diabetes and endocrine disorders, kidney disease, chronic illness, infectious disease and acute and rehabilitative care. She is a member of the American Academy of Nurse Practitioners and the American Medical Directors Association (AMDA).
Ms. Arnett primarily provides care to elders at Green House Cottages of Carmel and to patients at Options Behavioral Health, in addition to her work with Kidney Professionals of Indiana.
Ms. Armstrong completed her Bachelor of Science degree from Marian University and then completed her Physician Assistant studies at Butler University in Indianapolis. She previously collaborated with the Hospitalist team at IU Health Methodist, where she provided care across the continuum from admission, to patient evaluation and treatment.
Ms. Armstrong primarily provides care to residents at Evergreen Crossing & The Lofts.
Adult-Gerontology Primary Care Nurse Practitioner
Ms. Miller-Smith obtained her bachelor of science in nursing from Purdue University and her master of science in nursing from Indiana University, Indianapolis. She worked as a registered nurse for over 25 years at IU Health before obtaining her nurse practitioner license. Ms. Miller-Smith’s wide range of experience is the product of her time as a surgical nurse both in the hospital and outpatient settings, as well as in in private practice.
Ms. Miller-Smith primarily provides patient care at Options Behavioral Health and Indiana Veterans’ Home.
Family Nurse Practitioner
Ms. Patterson received her Bachelors and Masters of Science in Nursing from the University of Indianapolis. Ms. Patterson has a wide range of expertise with almost a decade of RN experience in the acute care setting, including Burn ICU, pre-op and recovery room, Surgical/Trauma ICU, and open heart and vascular progressive care.
Ms. Patterson primarily provides care to residents at Harcourt Terrace Nursing & Rehabilitation and Spring Mill Meadows.
Adult-Gerontology Nurse Practitioner
Ms. Anderson holds degrees from IU School of Nursing, Indiana Wesleyan University, Ball State, and Sharon General School of Nursing. She brings more than 30 years of nursing experience to Hamilton Medical Consultants, including her wealth of expertise in hospitals and as an educator. She is board-certified in gerontology and has experience in pain management and palliative care.
Ms. Anderson provides care for patients at Options Behavioral Health as well as back-up coverage at the skilled nursing facilities with whom HMC contracts.
Family Nurse Practitioner
Ms. Muddamalle has been a healthcare professional for more than 25 years. Her holds a Bachelor of Science degree and a Bachelor of Education degree from Osmania University in Hyderabad, India. She obtained her nursing degree from Rush University College of Nursing, Chicago, Illinois and her master of nursing degree from University of St. Francis College of Nursing and Allied Health in Joliet, Illinois. As a clinician, she has experience in long-term care facilities, hospitals, and wellness centers, and is skilled in managing acute and chronic conditions. Her volunteer efforts during mission trips to India focus on wellness awareness and disease prevention.
Ms. Muddamalle provides care to HMC’s patients at Options Behavioral Health.
Adult Nurse Practitioner
Ms. Bell is an Adult Nurse Practitioner who received her Bachelor of Science in Nursing from Marian College and her Masters of Science in Nursing from Ball State University. She has vast RN clinical experience in the acute care settings, including interventional radiology, nuclear medicine (cardiac testing), trauma, emergency unit care, cardiac progressive and surgical care, and endocrinology and dialysis patient care.
Ms. Lutes serves as the leader of operations and legal services for Hamilton Medical Consultants and Kidney Professionals of Indiana. She obtained her Bachelor of Science in Biology from Indiana University and her juris doctorate, cum laude, from Indiana University – Indianapolis. Prior to joining HMC and KPI, Ms. Lutes worked for almost 15 years in private practice law, concentrating on insurance defense coverage and litigation matters. She handled state and federal trials, hearings, motions practice, mediations, and arbitrations. She is a member of the Indianapolis Bar Association and Indiana Medical Group Management Association.
Ms. Kress has been with the KPI and HMC businesses since 2011, where she serves as the Office Manager. Originally from Indianapolis, she graduated from Indiana University with a degree in English. She also has a degree in Theatre Arts from Indiana University. Karen worked for several years in the not-for-profit arts sector.
With more than 16 years of patient experience in healthcare, Ms. Hampton joined the HMC and KPI teams in 2017. She holds an Associate of Science degree in Health Sciences and is able to sit for her Medical Office and Billing Specialist certification. Ms. Hampton serves as the medical scribe and assistant at Harcourt Terrace and Spring Mill Meadows. She also serves as the medical assistant for Kidney Professionals of Indiana at both the Fishers and Carmel locations.
Ms. Lawrence joined the HMC team as a medical assistant in 2017 following many years of experience in patient care and administration in the hospital setting. She primarily works for HMC at Evergreen Crossing & The Lofts.
Ms. Schwegman holds a psychology degree from Indiana University. She primarily serves as the front-line medical assistant at the Fishers office for Kidney Professionals of Indiana, and also assists with the behind the scenes administrative tasks for HMC at Green House Cottages of Carmel.
