HMC Medical StaffHamilton Medical Consultants, LLC was established by its founding partners, Drs. Anzaldua and Bustamante, as a way to effectively and efficiently provide primary-care, specialty-care, and long-term acute care services at facilities in Central Indiana.

Hamilton Medical Consultants includes a group of multi-specialty physicians and advance-practice providers with experience in high-acuity inpatient care, outpatient and post-acute care. We are skilled to provide the continuum of care each patient requires on his or her path to recovery, from the hospital setting to post-acute.

Hamilton Medical Consultants partners with select post-acute facilities to provide a level of oversight that is unprecedented in the market.

We collaborate with multiple subspecialists to produce the highest level of post-acute care. We constantly strive to provide the highest value in post-acute health as we target the right level of care, at the right time, for every patient.

Born and raised in Texas, Dr. Daniel Anzaldua has called the Indianapolis area home since 2003. His undergraduate training was completed at St. Mary’s University in San Antonio, Texas. He then attended medical school at Saba University School of Medicine.

Dr. Anzaldua completed his Internal Medicine residency training at St. Vincent Hospital in Indianapolis. After residency, he did his General and Interventional Nephrology Fellowship at Saint Louis University in St. Louis, Missouri. After completing his training, he provided specialized locum tenens nephrology services to rural and veteran’s hospitals across the country.

Dr. Anzaldua now focuses on providing high-quality care to patients in Indiana.

Born and raised in Michigan, Dr. Peter Bustamante attended Indiana University Bloomington as an undergraduate. He then obtained a Master’s Degree in basic science research at Loyola University of Chicago followed by medical school at Saint Louis University School of Medicine.

Dr. Bustamante returned to Indiana for Internal Medicine residency training at St. Vincent Hospital & Health Services. He went on to complete his Nephrology Fellowship at Indiana University and Interventional Nephrology training at Saint Louis University and is board certified in Internal Medicine and Nephrology by the ABIM. He also holds Vascular Access Certification from the ASDIN.

Dr. Bustamante has been practicing General and Interventional Nephrology in Indiana since 2008. In partnership with Dr. Daniel Anzaldua, the two started Kidney Professionals of Indiana (formerly Hamilton Nephrology, LLC) to serve the needs of patients with kidney disease and hypertension in central Indiana. In addition, they founded Hamilton Medical Consultants, LLC to focus on post-acute care medicine. Working with key partners in healthcare, they have developed and implemented novel systems and approaches to improve the care of patients following hospital discharge. Their efforts have resulted in a dramatic reduction in hospital re-admissions and improved the care of post-acute patients.

Dr. Thomas Armstrong grew up in Indianapolis, Indiana, and attended Earlham College in Richmond, Indiana, for his undergraduate studies. He then obtained his medical degree from Indiana University School of Medicine, following by a Family Practice internship with Indiana University Medical Center and then an Internal Medicine Residency at St. Vincent Hospital. Dr. Armstrong is board certified in Internal Medicine and in Geriatric Medicine.

Dr. Armstrong has practiced in Internal Medicine since 1987 in a primary care office setting. As well, he serves as a hospitalist with St. Vincent and has managed nursing home residents his entire professional career.

Dr. Armstrong and his wife live in Westfield, Indiana, and have grown children.

Jennifer grew up in South Bend, Indiana, and then attended Indiana University, where she obtained her Bachelor of Science in Nursing. She then attended Indiana University at the IUPUI campus for her Masters of Science in Nursing and Adult Nurse Practitioner degrees. Jennifer specializes in chronic illness, diabetes and endocrine disorders, infectious disease, and acute and rehabilitative care. She is a member of the American Academy of Nurse Practitioners and the American Medical Directors Association (AMDA).

Jennifer lives in Fishers, Indiana, with her son, Jackson, daughter, Grace, and German Shepard, Vixen. She enjoys reading, as well as watching all sports, especially IU basketball.

Kara D. Bell, ANP-C

Kara is an Adult Nurse Practitioner who received her Bachelor of Science in Nursing from Marian College and her Masters of Science in Nursing from Ball State University. She has vast RN clinical experience in the acute care settings, including interventional radiology, nuclear medicine (cardiac testing), trauma, emergency unit care, cardiac progressive and surgical care, and endocrinology and dialysis patient care.

Kara lives in Lizton, Indiana, with her husband, Chuck, and her daughter, Lexi. Kara enjoys running and she loves to watch the Indianapolis Colts and the Butler Bulldogs.

Tara grew up in Lafayette, Indiana, and obtained her Bachelor of Science in Biology from Indiana University. Following undergraduate school, Tara obtained her Juris Doctorate, cum laude, from Indiana University – Indianapolis. Prior to joining Hamilton Medical Consultants, Tara worked for almost 15 years in private practice law, concentrating on insurance defense coverage and litigation. She handled state and federal trials, hearings, mediations, and arbitrations. She is a member of the Indianapolis Bar Association, Indiana Medical Group Management Association (IMGMA), and Executive Women in HealthCare.

Tara lives in Indianapolis, Indiana, with her husband and their two young children. She is an active volunteer at her church, school, and local charitable organizations.

Karen has been with Hamilton Nephrology since 2011. Originally from Indianapolis, she graduated from Indiana University with a degree in English. She also has a degree in Theatre Arts from Indiana University. Karen worked for several years in the not-for-profit arts sector.

Karen and her husband, David, have five children. They enjoy volunteering with their church and schools, in CYO athletics, and with the Irish Dancers of Indianapolis.

Nicole grew up in Indianapolis, Indiana, and attended IUPUI. She joined Hamilton Nephrology as a Medical Assistant in December of 2013.

Nicole lives in McCordsville, Indiana, with her husband, Mark, sons, Evan and Cameron, and daughter, Isabelle. Nicole enjoys traveling with her family, volunteering at her children’s school, and attending concerts, Colts, Pacers and Notre Dame games.
